Metropolitan Region I Cardiovascular Disease Risk Screening and Education
An Effective Practice
Description
In an effort to promote and protect health and wellness, Louisiana's Region I established cardiovascular screening for all patients seen in their four health units. The clinics primarily perform Women Health services and screen for STDS and TB. It was determined that this was an excellent place to screen patients for cardiovascular disease who might not normally see a regular physician because of an inability to pay for services. All patients are screened, and if needed are referred to primary care physicians who have agreed to take these patients.
Goal / Mission
The objectives of the Cardiovascular Disease Risk Screening and Education program are as follows:
-To identify and provide early intervention for those with or at risk for untreated or uncontrolled hypertension
-To screen and identify those at risk for cardiovascular and renal disease
-To educate those found at risk for CVD
-To provide early chronic disease intervention
-To facilitate access for long term chronic disease care with partner sites
-To identify and provide early intervention for those with or at risk for untreated or uncontrolled hypertension

-To facilitate access for long term chronic disease care with partner sites
Results / Accomplishments
100% of patients who receive services at the health units are screened for Cardiovascular Disease and receive some level of intervention. Thus, far this has lead to increased follow-up for patients with Cardiovascular Disease risk factors.
